Informed Consent Form and Social Behavioral Protocol Template Updates

The USC IRB has updated the Informed Consent Form Template and Social Behavioral Protocol Template.

Links to both clean and tracked changes of the templates are provided below and available on our Forms and Templates webpage.

Informed Consent Template Summary of Changes
  • Added instructions regarding Key Information (does not require duplication)
  • Added guidance and template language for studies that include American Indian and Alaskan Natives as a targeted subject population.
  • Updated DoD guidance and template language for studies that involve DoD funding, and/or target Federal employees including DoD personnel, as a subject population.
  • Updated consent to include upcoming NIH Data Sharing requirements
  • Minor editorial changes (inconsistent formatting and language)
